How do I create a "form". I have a certificate-like document on ..

The document I have to print on needs name and address fillins in one place,
a date elsewhere and a signature.

How do I set it up so that these elements appear in the right place on the
document, beside the appropriate pre-printed copy?

Use a ruler and practice on plain paper till you get it right.

More than likely you will want to use a table with bottom borders in the
desired cells where names go.
