how do i create a questionnaire with multiple choices and one correct answer
that gets highlighted when clicked upon. also to send it back as an
attachment?

Word doesn't provide any functionality of this sort. You could probably achieve
it using macros, but then you have the problem that the user's machine would
have to allow the macros to run.

If you and the recipients all have Office 2003 you might consider using
InfoPath for this task.
