How do I create a survey in an e-mail to send to many people?

I would like to survey a select number of our clients with a generic survey
with given answers (strongly agree, agree, etc.) How can i do this using
outlook without using an attachment? (i would like to place the question in
the body of the e-mail so that the client may respond without exiting the
e-mail, and then be able to easily collect the data upon the e-mails return)

Outlook is not a suitable tool for conducting a survey like this. Consider using a web page instead.

You can design your own Outlook form to customize it like a survey... from
the Main Menu select Tools/Forms/Design a form..

But it's highly unlikely that such a form will work as you expect it to for outside recipients.
