how do i find a basic sign in sign up sheet/ template?

Meeting Sign in Sheet

I'm very frustrated with not finding what i'm looking for. I just need a
real basic "[meeting] sign in" sheet with the person's 'name', 'date',
'meeting' and 'time'. The purpose for me to gather the attendee's
information and signature is to proove to the coueet that I was at these
meetings. Can I make my own template or choose an already existing one?
Which is the easiest way to go about this?

I'd make my own instead of fussing with something that either had too much
information or the wrong info. All you need is a basic table. Five minutes
and you're done.
