How do I make a CD booklet?

I'm making a CD and found template to make the CD cover and the back of the
CD case, but I'm not sure how to make the CD booklet. If someone could let me
know that would be great!

Page setup, booklet, landscape, width 4.75, height 4.75

This setup will print on letter size stock with crop marks.
