How do I modify the default Contact form

I want to be able to add a tab that I can add various text boxt fields to the
default contact form. I want to be able to select new contact and have it
popup with my new form. Any help would be great. Thanks

FYI, there is a newsgroup specifically for Outlook forms issues "down the
hall" at microsoft.public.outlook.program_forms or, via web interface, at
