How do I Print Contacts in detail view and include all the field n

When I print my address book, The field names like home address, business
address, and other address don't show beside the data, while other field
names like Phone, Notes, Modified etc do show up. I would like all the field
names to print beside the information it represents. Help! I have an old
print out from Jun 04, and I did have it print that way, but cannot remember
what I did to make it work!
