How do I remove a published custom form in Outlook 2003?

I would like to remove a custom form that I published so it no longer
displays. How might I do this?

Tools/Options/More/Advanced Options/Custom Forms/Manage Forms: Click one of
the top buttons, select where the form is published, and click Delete.
