How do I set up option buttons to be clicked and selected?

I am new to these complicated word functions. I am trying to set up a
telephone consultation template for my pediatrics practice where people can
just click options (e.g. fever, cough, etc.). Then they can e-mail them into
the office so we have documentation (in this world of medical liability, it's
a must). I would like to have option buttons and scroll down options. I can
create them, but I don't know how to get them functional (they just sit there
unresponsive). Any help would be much appreciated.

once again, this seems to be the wrong group,
It is about programming Word
using visual basic for applications.
Maybe you should try microsoft.public.word.docmanagement
or
microsoft.public.word.newusers
