how find a Statutory of Declaration Form

I need to fill out a statutory of Declaration form and was wondering if
anyone knows if there is one on the web or any where that I can uses and
print out after I fill it out.

this group is about developing software in vba... try another group, any
other group would probably be more on topic than this one.
