How quit having [Spam] appear in message box of my sent emails?

Outlook 2003 Service Pack2. When I send emails-- even to one person -- they
often are received by the recipients with [SPAM] IN the subject box. my
messages then seem to often go to the recipients junk file folder.
Even when a person sends a message just to me [SPam] appears in the subject
box. When I reply or forward I erase the [SPAM] but it probably appears
again.

Can you tell me what's wrong?
Are my security settings part of this problem? If so, how do I specifically
adjust that without weakening spam defenses..
I use Comcast high speed internet.

someone's spam filter is adding it - probably the recipient's, but it could
be yours if you use something other than the one built into outlook.
