The rest of my query was the word form. This is the first time I have been
here and apparently i did something incorrect as it did not finish the
question. Honestly, I like to have a template and make changes that apply to
what i need. I have been unable to locate something simple. i do not want any
financial assistance - that is just the title of the form i need to produce.

You probably do not want a visual basic for applications answer. When
dealing with numbers that depend on each other, you are much better off
using Excel. If you need to do this in Word, you may want what Word calls an
"online form." Check this in help. For more about online forms, follow the

Chapman's series of articles. You may also want to look at
