How to develop a simple survey scripts

Hi,

Appreciate if anyone can point me to where can I find sample (and simple)
ASP scripts for a short survey.

This is the type of survey with only 2-4 questions, as seen on many sites,
and visitors can select one and click to see the result.

My hope is that this is not a complicated programming task.
