I am looking for a template regarding wrong bill sent to me

I keep getting invoices from UPS telling me that I owe them money. I have no
idea why they have an account in my name with a number but a shipment goes
from Mexico to Texas every month and they bill me. Numerous e-mails and phone
calls have not worked.

If a phone call hasn't worked, I can guarantee you that a letter won't. You
need to call again and speak to the supervisor.
