I need a template to acknowledge receipt of a letter

I am taking a letter into my bank and I need a form for them to sign to
acknowledge receipt of it. Does anyone know a good template I could use. I
have access to Office 2003 and 2007.

When I've need something like this, I just take a copy of the letter and
have them sign it "received by...<sig> & the time & date. They may also have
some sort of company stamp they could add to that.
