Icons in phone list definitions?

I have not been able to find any description of what the different icons mean
that show up in the left column of the contacts phone list view. What are the
meanings of the various icons that show up?

Most of my contacts show a note with a pin through it. Three showed up today
using the same icon as the "new contact" icon in the toolbar.

I am using Outlook 2002 and have created a custom form for my contacts.

Sounds like you didn't set an icon on your custom form on the (Properties)
page, so it's using the default pushpin note icon.
