A quick list from memory:

1) The projector losses one of its colours so everything looked green
2) The previous presenter set up my landscape slides in a portrait format in
their laptop
3) The laptop was incorrectly plugged in - went to sleep while my attention
was diverted
4) Background program popped up
5) An add in slows PowerPoint to a crawl
6) Some pictures were not inserted into the ppt file so I lost them when I
copied ppt to another PC

- presentation designed on ppt 2002, played back on ppt 2000

In addition to the other suggestions you've received --

*music won't play
*video won't play

(dont' know that I'd want to try to cover those in a short presentation,
though -- especially the video one)

*images or graphs don't show up in the file (happens in PPT 2003 -- apply
latest Office SP to resolve)

*computer crashes during presentation
