I'm looking for a template for detailed manufacturing instruction

I need to write detailed manufacturing instructions that will give step by
step instructions to the operator on how to build the product. There needs to
be a header in addition to the detailed instructions. Required fields for the
header include:
Title
Instruction Number
Date
Revision
Page "X" of "Y"
There should also be a sub-header that includes:
Equipment Required (machines)
Tools Required
Inspections Equipment Required
Inspection Tools Required
The next section should list the parts or components needed and include:
Part Number
Drawing Number
Description
Quantity Required
The balance of the sheet will be the detailed instructions themselves and
will be in a numbered format (i.e. 10, 20, 30.....). The template should
allow for the insertion of several photographs.
