"image.pdf" attachment added to message without my knowledge

I saw that there was one other post about this issue, but no
resolution. I did use the attachment button to attach a PDF called
"contract.pdf". The client I was sending it to called to ask why
someone else's name was on the contract. I looked at the file attached
to my outgoing message in sent mail and it was the correct file, with
their name on it - I couldn't figure out what they were talking about.
They sent the message back to me and there I saw the attached
"image.pdf" which I did not create and do not have on my hard drive
(other than in the Attachments folder now that he sent it back) - that
file was in addition to contract.pdf.

What's happening here? It's very disturbing to think I could be sending
things to people without any knowledge of it. During the first round of
confusion I thought that perhaps I hadn't saved revisions to the PDF on
my computer and that accounted for the discrepancy.

I hope someone can let me know under what precise circumstances this
can happen. I assume it's specific to Entourage.

I haven't seen this before, but it is possibly related to database
corruption, or possibly even file system corrution

Office vX:
Hold down the option key when starting Entourage and take the first option
('typical rebuild'). If this doesn¹t resolve the problems, then try an
ŒAdvanced Rebuild¹.

Office 2004:
You still call the rebuilder by holding down Option key at launch, but it
now brings up a separate app "Database Utility" (which could be launched by
double-clicking of you wish) that lives in the Office subfolder.

First of all, if you have any Office apps open, a dialog comes up that
offers to quit them.

Then you get a window with a list of your identities to select from, and
four options:

1) It has a new feature "Verify database integrity" option: this should
reduce unnecessary rebuilding. Apparently it's quite sensitive. And see 4)
below.

2) "Compact the database". (Like X's "Typical rebuild").

3) Rebuild the database. (Like X's "Advanced Rebuild")

4) "Set database preferences". Actually there's only one pref: you can turn
on "Verify database in background "which means "run automatically" and let
you know if your database needs rebuilding.

To check for file system corruption, run 'disk first aid' in your utilities
folder.

Thanks Barry. I did check the database and it passed the integrity
check. Do you recommend rebuilding the database anyway? I also ran disk
first aid on the hard drive and found a few errors, which I fixed. (I
have a G5 less than one year old). I hope that solves the problem but I
don't really know how to test it.
