Importing 800 addresses to E 04

We have a group of 800 addresses that we need to import to E 04 as a group.
Have read Paul's suggestion from way back when, but on import from .csv end
up with no names or emails in the proper places.

??when remapping fields on import?? What is supposed to happen when things
are remapped?

bother you all!!!
