Importing .csv from Outlook to GMail

Whenever I import my .csv from Outlook - 501 entries, I appear to be
losing entries in the Gmail and Yahoo Contacts.

GMail shows only 488 imported - 13 lost. The address book shows only
486 - another 2 lost.

I tried it in Hotmail - all 501 imported.

I tried it in Yahoo - 493 imported.

I have tried with both DOS and Windows .csv

Help!
