In word I would like to insert a tick box

I am having difficulty inserting a tick box in a word document. I am
producing a true / false questionnaire and each time I try to insert a box
the numbered section seems to jump out of sequence.

example

Consider the following True False
1 question one insert tick box insert tick box
2 question two
and so on

The best way to construct a questionnaire is to use the table format.. you
can keep the lines or make them invisible if you so wish..

Thanks for your response, however I'm not sure how to make it work in a
table. I was simply looking to insert a square in two columns for which the
person asnwering the questionnaire could tick the appropriate true or false
box. I have looked in the insert symbol section but could not find a square
to insert. Any suggestions?

If you go to Table - Insert, you can create a table two columns wide and
three rows deep.. enter your headings into the top two columns..

Type your first question into the box below the Heading.. use the TAB key to
get to the next box.. go to INSERT - Symbol and insert the little box
symbol.. it should be the first choice in the symbols window..

OK.. first to rows are full.. with your cursor still in the box with the
little checkbox, press the TAB key to create another row, and just fill in
your questions.. the little checkbox symbol can be copied and pasted where
necessary..

Columns and rows can be sized by dragging lines in and out.. leave the
gridlines showing while you work on your table/form..

It's that easy.. if you do not want lines showing, highlight the entire
table and then right click to get to Table Properties.. click on 'Borders
and Shading'.. look to the left and you will see the options.. a few minutes
messing here and you will be an expert table and form creator..
