Job complettion and signoff

HI all,

I am looking for a template that will enable me to get a signiture from a
client on sucessful completion of a programming Job.
What I probably am looking for is something with a description of tasks
performed, hours it took, perhaps a description of problems encountered, and
then a final signoff from the client that this is indeed exactly what I did
