Inside the Inbox, or at the same level as the inbox?
Entourage will create a folder called 'junk E-mail' at the top level of the
account folders if it doesn't find one there already - I believe that it is
intelligent about this and will use an existing one if it is called 'junk
mail', junk' or something similar.
One created, this folder cannot be deleted (it is a 'special' folder like
the inbox, draft, outbox, sent items & deleted items folders. These folders
have special id numbers from 1 to 6. All other user-created folders start
from id 100. However, this folder should not be inside the inbox. If you
have a folder inside there as well as one just below the 'deleted items'
folder, could it have been created by your ISP? Is this an IMAP account?

Yes, I have a junk email folder at the top level that is undeletable. This
'new' junk email folder just appeared out of nowhere inside my Inbox in the
last week or so. I've tried deleting it but it reappears. Nothing has
changed about my email account that I know of. Our email is handled by an
exchange server. I suppose something could have been changed about the
server but there's not really any way for me to know that.

If it keeps reappearing it's almost certainly server related.
