Junk mail protection Entourage 2004

I get junk mail in inbox for items not in Address Book or Safe domain and I have Exclusive level of protection checked. These are also emails not in my Mailing list manager. How do they get through and is there anything else I can do for key words in the subject line?

Is it possible that these messages were marked as junk by the JMF, but a
rule was triggered by them which moved the message, or caused it to be
un-junked?

All you can do on subject lines is set up your own incoming mail rules
(which gets tiresome after the first few thousand rule changes) or use a
solution live SpamSieve, which is adaptive (it learns) and very effective:

If they are not in your Address Book, they could land in your Inbox. Safe
domains aren't as reliable as adding the contact to your Address Book.
Assign categories to all contacts. This way you can manage your contacts
easily. Some users are reluctant to add lists and newsletters think it will
clog up their Address Book. With categories it's easy to see only the
contacts you want.
