I presume that you want to do this in Word, which is MS
Office version of WordPerfect.

In Word create the text of the document including any
Tables. You do not have to leave additional spaces at this
point.

Choose, View, Toolboxes, Forms. The box you require is a
text box that looks like ab|. Insert this where you want
the variables to go. and close the toolbox when there are
all added.

The text boxes cannot be used until the document has been
protected. Choose Tools, Protect Document and choose Form
in the OPtion dialog.

Save the document.

You have now created a form document in Word.

If you want to do this in Excel (This is an Excel Forum)
There are similar options including some that involve VB.
If this is the case post again giving more details.
