Looking for a materials form/templet

I'm looking for a templet form to use to return defective goods (Excel
format). Something that already exists that includes fields or info like
initiator, supplier PO#, reason for return, disposition, shipping info,
etc... You know, all the necessary ingrediants. Can anybody out there help

as you have stated the info you want, why can't you design your own?
