looking for a template for daycare for a sign in sheet weekly

[A Wonderful Daycare Provider in MI]

I am looking for a simple sign in sheet for 5 days for my daycare. It would
have a place for the child's name, then Monday thru Friday with columns that
have a sign in and out section by the childs name.
