Looking for an Insurance Settlement letter

I was in a car collision and want a letter requesting a substantial
settlement for my injuries... this would be against my own carrier as the
other person had no insurance. All medical agencies are biling my private hc
provider. And if they are out of network I will be responsible so I need a
settlement to cover this and future therapy which is still continuing.

I have word 2000
