Under OS 9/Office 2001 the correct location for your identity folder is in:

<Startup Disk:Documents:Microsoft User Data:Office 2001 Identities:

Under OS X/Office vX it's:

~/Documents/Microsoft User Data/Office X Identities/
(where '~/' is your current login's 'Home' directory

Make sure you put your main identity folder in there, then if you still
can't see it when you restart Entourage, try to change identities (it's
under the File menu) and see if your identity shows up in the list.

Look in the folder ~/Documents/Microsoft User Data/Office X Identities

Is there a folder in there with the name of your original identity?

If not, then you probably have some disk corruption and should try
diskWarrior or some similar recovery utility.

If the folder is still there, look inside it - there should be several files
in there such as:

Database
Database Cache
Mailing Lists
Rules
Signatures

If all that is there, then the files you need are still in place.

Now, start up entourage, and under the entourage menu, choose 'Switch
Identity...'. What shows up under that window? Does the list of identities
match the folders in the Office X Identities folder? Can you select the one
you want?

Please report back.
