Mail Merge for a Directoroy

I have four fields in Office 2003 Excel. In Word 2003 the main document
consist of house number and street (street address), them (the homeowners),
Lastname (the homeowners last name), phone (the homeowners telephone number.
When I do a one-at-a-time merge everything appears as it should.
1300 Cambridge Place ZZZZZZZZZZZZZ Murphy
XXX-XXXX
Names and phone number changed. But see how well it lines up?

When I do a merge of all members this is what I get:
1200 Cambridge Place Bob & Virginia hhhh
xxx-13911201 Cambridge Place Tom & Geri hhhhhhh
XXX-8764YYYY Cambridge Place Cecil & Connie
The first line is as planned, but notice on the rfight how the second line
house number follows in with the first line phone number. The second line
starts with out the house number and the first names don't line up in the
place marker, etc.

I have got to be doing something wrong but for the life of me I c an't
figure it out and neither have the experts.

In setting up your directory, after you put the fields in, press the Enter
key ONCE. Then run the merge.
