missing buttons when inserting contact into word - revisited

I had previously posted on this matter, but now I have some more information
and I am hoping that someone will be able to help.

The problem with the missing radio buttons in the Insert Address dialog box
(named "Select Name") in Word had previously been noted with Outlook 2000:
OL2000: (CW) Option Buttons Missing Accessing the Outlook Address Book from
Word Article ID: 240629

However, when I replaced the supposedly missing file: schdmapi.dll and run
the repair, it doesn't correct the problem.

Has anyone else noted this problem with XP and Office 2003? I didn't have
this problem with Office 2000.

Any ideas on how to fix it???
