missing columns in Outlook 2003

In some views of the Inbox, the user has no columns showing at all. There's
not even a space where you can drag columns to (from Field Chooser).
Switching views, some of the columns, and some have none. I can't find any
option that would remove or restore the entire column bar. Originally, the
Messages with AutoPreview view showed no columns, but Messages did. While
trying to fix Messages with AutoPreview, something caused Messages to have
the same problem.
The frame shows
Inbox
Group by Box
Individual Messages
There should be another row in there with column headings.

Try starting Outlook one time with the /cleanviews switch.

Start->run->outlook.exe /cleanviews - note the space before the /.
