Modifying the Contacts Form

I am trying to modify the contacts form so that it will help me keep track of
purchases by contacts and provide me with a list of birthdays, anniversaries
etc....

How can I modify the form so that I can fields like a spouses birthday,
stock # of a purchase, my commission amount, etc...
