Multiple choice questions

I use PowerPoint 2000 in Windows 2000. I am trying to develop multiple
choice questions based on microscopic images. The idea is to display an
image and the multiple choice question displayed simultaneously. Ideally,
the ID of the test taker and final grade should be saved to a file. Are
there any tutorials, templates, etc available?
