need form for someone to take over payment on vehicle

could some one make me a form so i can have someone take over paymenton my
vehicle

What names would you like to be entered into the appropriate places?.. what
make of car?.. what mileage?.. what colour?.. how old is the car?..how is
the payment to be made?.. what is the payment frequency?..

Would you like full colour, or will plain monochrome do you?.. font
preferences?.. do you want it formatted for letter or legal size paper?..

I'll wager you thought that this will be a simple task..
