Newbie form design

Hi,

I want to create a pop-up dialogue box that asks users
which of 8 modules they would like to include in a user
guide. 8 check boxes linked to either insert bookmark from
other file macros, or includetext fields. Then use an on-
exit macro to read the check boxes and create the user
guide ?

Is this possible. Where do I start cerating this user
form. Is it done as a table / frame and saved as .dot ??
