Outlook 2003 Crashes when Attaching Files

I have a client who recently ugraded from Office XP to Office 2003 - Now each time she tries to attach any file type to Outlook, Outlook crashes and this error appears in the log. Looks like the only add--in that might matter is Symantec Antivirus Corp Edition 8.1. I have tried with the add-in deselected and with Exchange Protection disabled. I have been able to find this several places, but no fix for the Outllib.dll issue.

At what time of the attaching process does Outlook crash? Does it also crash
when someone else is logged on to that computer? If not you might want to
recreate the mail profile. Clearing the Deleted Items folder, your
IE/Explorer History and Temp files from within Internet Options never hurts
either.

It crashes within a second of clicking the file to attach -- File size or
file extension seem to have no affect. It also does not seem to matter if
other software is open or not or if it is done right after a re-boot. The
mail profile has been changed and no fix.

spoke to microsoft support. it was a com add in tools --> options -->
other --> advanced --> com ad-ins

it happened to be made by a company called delta something or other.
it was installed well before outlook, and integrated itself. I
imagine other programs can do the same.

I hope this helps someone.
