Outlook 2003 video mail add-in error

I installed the video mail add-in for Outlook 2003.
When I open the video message window all seems to be
fine, the camera is showing the image, but when I try to
record the window flickers and I recive the following
error:
The oeration is stopped due to unexpected error.

When I click OK it closes.
I have tried to re-install with the same results.
Any ideas?
