Outlook & Junk email - where to go?

I'm running Outlook 2003 and have set security to High, safe lists only.
I've reviewed my safe list, block senders and contacts. However
approximately 1/3 of all spam still appears in my inbox. Clearly these
senders are not in my safe senders list nor my contacts - I've even
turned off automatically including contacts.

Several days of google have proven fruitless.

Is there a hidden email rule that's been corrupted? I really am not
eager to re-image my machine.

I use SpamBayes in addition to Outlook's filters and I almost never get spam
in my in-box.

Post all replies to the group to keep the discussion intact. All
unsolicited mail sent to my personal account will be deleted without
reading.
