Outlook Reminders

After setting up a sub-folder under contacts and setup for
a follow-up call time, will not send reminder. This will
send reminder under the main contacts.

That's expected behavior. Outlook only fires reminders in default folders.
Check out Extended Reminders from www.slovaktech.com for an add-in that will
fire reminders in all folders.
