Does anyone have a template, or know how to, create a printable fax cover
page from Outlook for a contact?

Thanks, but how do I get Outlook to recognize the template? When I am in the
contact info and click send a letter, the template isn't there.

I have no clue, but in my opinion if you are sending a fax, then you need to use
fax software.

I'm sending a fax the old-school through a fax machine and need to print out
