Periodic freezes, lockups

I'm having a problem that I suspect is somewhat common with people with
similar configurations as me, but makes Entourage unusable.

I have Entourage 2004 11.1.0, and using it to talk to an Exchange 2003
account.

My mailbox is rather large - 291MB on the server, and my local database
is 791MB.

Periodically Entourage just "hangs" for 30 seconds or so. This seems
to happen each time my mailbox is being updated - I always see "Now
updating Inbox".

In Activity Monitor, I see Entourage reading data like crazy - in fact,
each time this happens it looks like about 790MB or so of data is read
from disk.
From this I'm thinking that every time a Mailbox update occurs,
Entourage seems to want to read the ENTIRE database, and in the
meantime all activity? That seems very odd.

This makes Entourage unusable for me since my mailbox seems to update
frequently - it's tough to tell exactly how often since the update
itself takes a very long time (but is usually invisible to me), though
I believe that - every time I switch away from the app and back, every
time I click on a different folder, and probably at other times too,
Entourage seems to kick off an update and at some point before it
completes I see this huge (up to 60 second) pause.

I've done everything I can think of to troubleshoot this - run all the
"database repair" options I could find, tried deleting everything and
re-setting up my mailbox, etc. Does anyone else have any ideas?

I don't use Exchange, but does that have a feature called "Live Sync"?
If so, I suggest turning it off. I've read that it can tie up your
computer like this. Instead, do a manual sync once in a while, when you
don't mind having your computer tied up for a bit (like a bathroom
break).
