Pop-up prompt questions - .dot format

Hi there,
I wanna have a .dot template Word document that when started prompts the
user with a number of questions to the user that when answered that info is
automatically transferred to pre-designated areas in the document. I.e. when
opening the document a question states: "What is your name?" when entered
into the prompt area and OK'd this info is then pasted to a set number of
positions inside the Word docuent.
How do I do???

You may want to ask this question in a Word newsgroup. This newsgroup
is for a different program, Microsoft Access, the database included in
Office Pro.
