PRE-MADE TEMPLATES AND "SUBMIT" FORM BUTTONS

I am in dire need of help...we purchased a pre-made
template for our business that already had 2 text box
areas(for email address and co name) and the"submit"
button. I don't want to create a new submit button- I'm trying to use the one that I have

I need to have these results emailed to me when they hit
the submit button but cannot seem to configure it
properly

Can anyone out there help? Im eager for a response as we
have already published the website and the component is
up there on our home page not working properly
