Printing contacts to Rolledex cards

I have tried every way possible to print these contacts to rolledex cards but
when I try to set up the mail merge to print them it scatters information all
over the place and does not put them into the proper format. Can anyone out
there help me out? I am about to pull out my hair on this one and it is
probably a simple fix.
