Protected Form - Coding Solution for Multiple Options?

I have quite a few forms that I need to have the ability for the user to
choose one or multiple options and have his/her choice(s) be inserted into a
paragraph. The doc will be protected.

Example:

Your request to access [insert one or more options below] has been denied
because clients and legal representatives do not have the right to access
this information, as mandated by the HIPAA Privacy Rule, 45 CFR Section
164.524 (a)(2). The decision to deny access to this PHI is not subject to
review, and is in accordance with the HIPAA Privacy Rule.

Option 1: Psychotherapy notes

Option 2: PHI compiled in anticipation of, or for use in, civil criminal, or
administrative actions or proceedings.

Option 3: PHI maintained by DHS that is subject to or exempt from certain
provisions of the Clinical Laboratory Improvements Amendments (CLIA) of 1988.

Option 4: PHI requested by an inmate, maintained by a correctional
institution, or by a provider on behalf of the correctional institution, if
obtaining the information would jeopardize the health, safety, security,
custody or rehabilitation of the individual or other inmates, or the safety
of any officer, employee, or other person at the correctional institution or
responsible for transporting the inmate.

Option 5: PHI created or obtained by a provider in the course of research
that includes treatment, if the individual has agreed to denial of access
when consenting to participate in the research. In addition, the provider
told the individual that his or her right to access will be reinstated when
the research is completed.

Option 6: PHI obtained from someone else, other than a health care provider,
under a promise of confidentiality, but only if access would be reasonably
likely to reveal the source of the information.

I would probably need to separate each selected option with a semicolon, and
insert the word "and" before the last option selected. Is there a
programming solution or a reference that would assist? I have tried
researching options with no success so far.

Any help would be greatly appreciated.

Given that your options are quite complex and may not be well known to all
users, I would have a macro run on entry to the formfield to display a
userform that contained a multi-select listbox containing the items.

Thank you so much! Those tips really helped. I was able to create a
userform in my protected doc that ran on entry of a form field and inserted
the different options into the field itself (separated by semicolons and the
"and" before the last option selected if there was more than one) so the user
could go back to the field if the options needed to be changed. The only
thing I have left is minor constraints that are pretty simple.

Thanks again... my coworker is *very* happy that I was able to cobble
together a solution.
