Raise request forms

I need a general raise request form that in NOT in the form of a
letter...Kinda like a evaluation/raise request..

Templates are generally used to format a document. It sounds like you're
looking for words. Or am I misunderstanding? I think your request is too
specific for anything you're going to find online. You're probably going to
have to work from scratch.

okI found an evaluation form that is almost perfect..I guess I just need an
addition on it that allows pay grade information on it. Most jobs I have
worked at when an evaluation was given it was FOR raise consideration...and
the form always had a place on it that gave what the person was making at the
time of the evaluation and if there was a raise considered and if so how
much...does that tell you more what I am looking for? I am no good at
creating these things myself which is why I am in here...thank for your help
