Report problems

I am running into a problem with my report. My database stores cases on a
monthly basis. I am trying to capture the following:
EleNum Element Finding NatCode
10 Relationship 1
11 Living Arrangement 2 4
12 Time Limits 2 3
13 Work Registration 1
14 Immunization 2 2

Some cases capture up to 3 findings for each element. How can I set up a
report to count the findings for example: relationship - finding1 finding2
finding3/ natcode1 natcode2 natcode3 for each element of each case.
