Resume form: please help

Hello,

I found this resume template file, that is a "form" you simply fill out the
input boxes, and then it sends the input to a "resume style template" which
you can see and print.
But there are no instructions. Can you help me set it up on my Website
please. I don't know at all what the first step is, and second I don't know
what to fill out in the "Submit" button variables ? Thanks everyone for
